Amica Clinical Consulting offers two types of FMLA mental health evaluations. Payment is due at the time of service.
Intermittent FMLA Evaluation — $450
This includes a 60-minute telehealth evaluation for adults who may need protected time away from work on an intermittent basis.
Intermittent FMLA may be appropriate when you are generally continuing to work but need leave for symptom flare-ups, panic attacks, trauma triggers, therapy or psychiatry appointments, depressive episodes, anxiety episodes, or periods when symptoms temporarily interfere with your ability to function at work.
Continuous FMLA Evaluation — $600
This includes a 90-minute telehealth evaluation for adults who may need a continuous block of leave (3+ days).
Continuous FMLA may be appropriate when you believe you need a block of time away from work to stabilize symptoms, begin or intensify treatment, recover from a significant mental health episode, or address symptoms that are currently making it difficult to sustain work.

Important: This evaluation type covers evaluation for FMLA leave only. Evaluations for short-term disability, paid leave, or state-based leave are more extensive and require more comprehensive work-capacity and functional assessment evaluation.
Not Sure Whether You Need FMLA, Short-Term Disability/Paid Leave, or Both?
FMLA generally protects your job if you qualify, but it does not automatically provide income replacement.
Short-term disability or paid leave (e.g. state based FMLA) may provide wage replacement if you meet certain criteria.
Some people need evaluation for FMLA only. Others need evaluation for short-term disability or paid leave (which also includes FMLA).
Can FMLA Cover Mental Health Conditions?
Yes. The Family and Medical Leave Act (FMLA) can provide up to 12 weeks of unpaid, job-protected leave to eligible employees who need leave due to a serious health condition, including mental health conditions. Mental health conditions can be serious health conditions and qualify for completion of FMLA paperwork when symptoms significantly affect your ability to work or require ongoing treatment. These concerns can affect concentration, attendance, emotional regulation, sleep, stamina, and the ability to keep up with work demands.

What is FMLA?
The Family and Medical Leave Act, or FMLA, allows eligible employees of covered employers to take job-protected leave for certain family and medical reasons. This may include a qualifying mental health condition that makes it difficult or impossible to perform essential job duties.
FMLA generally provides up to 12 workweeks of job-protected leave in a 12-month period. FMLA protects your job, but it does not automatically provide pay.
What is the WH-380-E form?
WH-380-E is a standard Department of Labor form used when an employee requests FMLA leave for the employee’s own serious health condition; it is used for the healthcare provider to provide FMLA certification.
Can I get FMLA for anxiety, depression, PTSD, or another mental health condition?
Yes. Mental health conditions may qualify for FMLA when they meet the definition of a serious health condition and interfere with your ability to work or require continuing treatment.
This may include conditions such as anxiety, depression, PTSD, panic disorder, postpartum depression or anxiety, OCD, trauma-related symptoms, or other clinically significant mental health concerns.
Is FMLA paid?
FMLA protects your job, but it does not provide pay by itself. Some employees use PTO, sick leave, vacation time, employer-paid leave, state paid leave, or short-term disability benefits while they are also on FMLA.
Your employer or benefits department can tell you what paid options may be available to you.
Am I eligible for FMLA through my employer?
Not all employees qualify for FMLA. Eligibility depends on factors such as how long you have worked for your employer, how many hours you have worked, whether your employer is covered by FMLA, and whether you have available FMLA leave.

My employer says I can get paid through short-term disability. If you recommend FMLA, will you also complete my short-term disability forms?
FMLA and short-term disability are different.
FMLA protects your job if you qualify. Short-term disability is an insurance or benefits claim that may provide partial wage replacement if approved.
Short-term disability forms usually require more detailed clinical information than FMLA forms and often have different approval/qualification rules. If you are applying for both FMLA and short-term disability/paid leave program, those evaluations are more extensive - please schedule the evaluation type that covers both evaluation needs.

How long can I take off work for FMLA?
Eligible employees may generally take up to 12 workweeks of FMLA leave in a 12-month period for qualifying reasons. Mental health leave may be intermittent or continuous when clinically appropriate and approved through your employer’s FMLA process.

Do you guarantee FMLA approval?
No. We provide a clinical evaluation and documentation of leave needs only when clinically appropriate. Your employer determines eligibility and approval under FMLA rules. Employee eligibility depends on factors like length of employment and company size - for example, individuals who have been employed with the company for less than one year may not be eligible - please check with your HR department to confirm.
Do you send forms directly to my employer?
No. Completed forms are provided to you through the secure patient portal. You submit them directly to your employer, HR, or leave administrator.
Do I need a diagnosis to get FMLA for mental health?
FMLA documentation generally requires clinical information supporting a serious health condition, functional limitations, and need for leave. During the evaluation, the psychologist assesses diagnosis or working diagnosis, symptom severity, impairment, and treatment needs.
Do you accept insurance?
No. We do not work with any insurance companies. Payment is due at the time of service and can be accepted via credit card through the patient portal. A receipt may be provided for possible out-of-network, HSA, or FSA use, but reimbursement is not guaranteed.
Can my therapist complete FMLA paperwork?
Maybe. Some therapists, psychiatrists, physicians, or other providers complete FMLA forms, but many do not because of time limitations, practice policies, scope of practice limitations, or documentation requirements. Do you provide treatment after the evaluation?
In some cases, yes. If we have availability and your clinical needs are within the scope of our therapy practice model, we may offer short-term therapy focused on stabilization, brief evidence-based intervention, coping skills, treatment planning, and recovery. When longer-term or specialized care is needed, we provide referrals whenever possible. However, since ongoing treatment typically involves weekly visits, many patients prefer to see a provider that is in-network with their insurance to reduce the financial burden - we can provide referrals to in-network providers, if needed.
