I Already Have a Diagnosis. Why Won’t My Healthcare Provider Write a Schedule A Letter for Me?
Disability: A diagnosis doesn’t always mean someone is disabled at work and your provider may not feel comfortable determining whether the condition is disabling.
Lack of Knowledge/Training/Expertise: Many healthcare providers are unfamiliar with Schedule A requirements, definitions, qualifications, etc. In most cases, if folks are unfamiliar, they refer out to someone more specialized.
Strict Requirements Associated with the Term Disability: The terms “disability” and “forms” often lead healthcare providers to think about the very strict requirements and definitions often associated with qualifying for social security disability. However, Schedule A is actually to help you get a job.
Organizational Limitations: Some providers just don’t or aren’t able to provide additional services outside of treatment of your health condition. For example, if you have an online doctor on a large platform, the platform may not compensate providers for the additional time needed OR may not permit providers to complete documentation other than regular notes about your treatment visit.
Compensation for Additional Services: If you use insurance for your healthcare treatment services, insurance typically does not compensate providers for their additional time to review records, complete forms, verify forms, etc. This leads some providers to limit the availability of these services.
How Quickly Can I Receive My Schedule A Letter?
When clinically supported and records are sufficient, documentation may often be completed promptly, sometimes within the same day or within 48 business hours.
How Do I Know If My Mental Health Condition Counts As a Psychiatric Disability?
You must be diagnosed with a mental health disorder/psychiatric diagnosis AND it must substantially limit one or more major life activities. Many mental health conditions are considered disabilities, such as Major Depressive Disorder, Bipolar Disorder, and Posttraumatic Stress Disorder (PTSD) as they tend to have significant impact on and substantially limit one or more major life activities. The ADA, as it pertains to mental impairments, defines disability as a mental impairment that substantially limits one or more major life activities.

Our Process
Book Online or Send us a secure inquiry message. Let us know what condition you need an evaluation for and what type of prior disability documentation you have, if any. We will respond to determine if we might be able to assist.
If moving forward, you’ll receive an email from the patient portal to complete consent forms, intake forms, and online questionnaires. Upload any records to be reviewed (we must review your records if you already have a diagnosis). You must complete all forms within 48 hours of scheduling in order for us to keep the reserved time on the schedule.
Review of Document/Intake: Your provider will review your documents in advance. If there is a question or if we cannot provide the service, we will send you a secure message with additional information or questions.
Log-In To Visit: Meet with psychologist for about one hour to discuss the disability.
Download Documents: You will be able to download completed documents often the same day and usually within 48 business hours.

Evaluation Fees
The fee for this service is $600 in most cases, IF you already have a diagnosis or established disability with available documentation. This fee includes your evaluation/consultation, detailed review of records, any questionnaires and scoring/interpretation, as well as completion of documentation (when deemed clinically recommended) for work.
If you do not have a diagnosis already, evaluations start at $1500 for conditions such as Major Depressive Disorder or PTSD. Please contact us to discuss your needs and type of evaluation needed. For example, evaluations for a diagnosis of a neurodevelopmental condition, such as ADHD or dyslexia, typically require more extensive evaluation and/or testing. This fee includes your evaluation/consultation with an added focus on identifying a diagnosis (if there is one), detailed review of records, any psychological testing or screening measures, scoring and interpretation, a brief report, as well as completion of documentation (when deemed clinically recommended) for work.

Will this guarantee I get the job I am applying for?
No. Our evaluation can only address whether clinical findings support Schedule A documentation. Hiring decisions remain with the employer.
How soon can I have my paperwork completed?
One of our major goals is to support people with serious or chronic mental health conditions access resources that will help them live a high quality of life. We can often provide you with evaluation results and completed documentation within 48 business hours, but no later than one week after your evaluation.

What type of payment do you accept? What about health insurance?
We accept all major credit cards, including HSA and FSA credit cards. You must make payment to us directly before or at the time of service. We do not accept insurance or work with insurance companies. In many cases, a visit only for a Schedule A letter is not considered medically necessary and would not be a billable service.
Do you offer evaluations for other mental health conditions or questions not listed on this page?
