You may know that something is affecting your work, education, emotional well-being, or daily functioning, but you may not be sure what type of evaluation you need. You may also be wondering whether comprehensive psychological testing is necessary, whether a more focused evaluation would be appropriate, or whether another provider may be better suited to address your concerns.

An initial consultation offers a personalized starting point.

Together, we will clarify your concerns, consider the questions you hope to answer, and discuss the evaluation or referral options that may be most appropriate for your situation.

You do not need to arrive with a specific diagnosis, test, or evaluation type in mind. The purpose of the consultation is to help organize your concerns, clarify the questions you would like answered, and identify a clinically appropriate direction.

  • Exploring documentation related to workplace accommodations or leave

  • Trying to determine whether therapy, testing, medical care, or another specialty service would be most appropriate

The consultation is a focused clinical conversation centered on your concerns, goals, and possible next steps.

Depending on your situation, we may discuss:

  • What prompted you to seek consultation

  • Brief psychological, developmental, educational, medical, or occupational history

  • Previous diagnoses, evaluations, treatment, or accommodations

  • Current treatment and medications

  • The questions you would like an evaluation to answer

  • Whether psychological evaluation/testing appears clinically appropriate

  • Whether a focused or comprehensive evaluation may be needed

  • Whether another type of professional or service may be a better fit

  • Anticipated fees, timelines, records, and next steps

The goal is to provide thoughtful clinical direction—not to automatically recommend evaluation/testing. This is a focused consultation designed to provide clarity and direction for further evaluation. It is NOT a specialized psychological evaluation or treatment/therapy appointment.

Following the consultation, the recommended next step may include:

  • A focused or targeted psychological assessment

  • An evaluation related to workplace functioning, leave, disability, or accommodations

  • An academic or high-stakes testing accommodation evaluation

  • A comprehensive psychological evaluation

  • Ongoing therapy

  • Consultation with a physician, psychiatrist, educational professional, or another specialist

  • Additional treatment, documentation, or records before an evaluation is scheduled

  • No formal evaluation/testing at this time

  • Referral to another provider who specializes in your area of need

Psychological evaluation/testing is not always necessary or appropriate. When another service may better address your needs, we will offer honest guidance and, when possible, help you identify an appropriate direction.

Our recommendations are based on clinical appropriateness and the information available.

NOTE: An initial consultation does not include psychological testing, establish a definitive diagnosis, or guarantee that an evaluation will be recommended or accepted. Recommendations and service decisions are based on clinical appropriateness, the referral question, available information, professional judgment, and the scope of services provided by Amica Clinical Consulting.

Possible Recommendations

Following the consultation, you will receive clinically informed guidance about the next step that may be most appropriate. Psychological evaluations are intended to offer specific questions. Each type of evaluation may use different tests/methods to answer the questions. For example, an evaluation intended solely to help with planning therapy/treatment will not address workplace accommodation or academic accommodation needs.

Recommendations may include:

A Specialized or Focused Evaluation Related to the Diagnosis or Treatment of a Psychiatric Condition

  • FMLA leave due to needing treatment and recovery time for a psychiatric condition

  • Short-term disability or paid leave due to needing treatment and recovery time for a psychiatric condition

  • A Diagnostic clarification evaluation for treatment planning purposes. A psychological evaluation may be recommended when symptoms are overlapping, persistent, or difficult to understand and your goal is to gain clarity on your diagnosis and treatment planning. This may include assessment of concerns such as ADHD, learning difficulties, anxiety, depression, trauma-related symptoms, OCD, mood symptoms, sleep-related concerns, or other clinical questions.

A Specialized Evaluation To Provide Opinions/Answer Questions Not Related to Treatment

  • Academic accommodations: to answer questions about how a psychiatric condition affects academics and the need for accommodations

  • High-stakes testing accommodations: to answer questions about how a psychiatric condition affects test-taking and the need for accommodations. This can include psychoeducational and neuropsychological evaluations.

  • Schedule A disability-related documentation: to answer questions about the presence of a disability for job opportunity purposes

  • Independent administrative or other evaluations: to answer other specific questions, often in relation to accessing a resource or benefit

Treatment or Medical Consultation

Your concerns may be better addressed through psychotherapy, psychiatry, primary care, sleep medicine, neurology, or another healthcare specialty.

Referral to Another Provider

When your needs fall outside the practice’s scope, we will provide honest guidance about the type of provider or service that may be more appropriate.

Is This Consultation Required?

No.

If you already know which Amica Clinical Consulting service you need and that service is available for direct scheduling, you may schedule it without first completing this consultation.

The initial consultation may be especially valuable when:

  • You are deciding between several services

  • You are unsure whether evaluation/testing is necessary

  • Your concerns do not clearly fit one evaluation category

  • You need help identifying the appropriate referral question

  • You want to understand the likely process and investment before proceeding

A consultation does not guarantee that an evaluation will be accepted, scheduled, or recommended.

Initial Consultation & Fees

The fee for the Initial Consultation for Psychological Evaluation or Testing is:

$450

Payment is due at the time of service.

The consultation fee covers the initial clinical appointment and recommendations regarding possible next steps. It does NOT include psychological testing, extensive records review, diagnostic clarification, documentation/completion of forms, or a psychological evaluation report.

If a separate evaluation is recommended, you will receive information about the proposed scope, expected process, estimated fee, payment requirements, and what is included before evaluation/testing begins. This may or may not be the same as our flat rate evaluations, since we can offer customized evaluations based on the initial consultation.

Amica Clinical Consulting is an out-of-network practice and does not accept insurance. Payment is due at the time of service. A superbill (this is a detailed receipt you can submit to insurance for possible reimbursement if you have out-of-network benefits) may be available upon request for possible out-of-network reimbursement if there is a psychiatric condition and the purpose of the consultation is for the treatment of a medical condition (e.g. evaluation to clarify diagnosis and treatment plan). Reimbursement is not guaranteed. Clients are encouraged to contact their insurance company directly to ask about out-of-network benefits, telehealth coverage, deductibles, and reimbursement requirements.

Preparing for Your Consultation

You are welcome to provide brief records that may help us better understand your concerns, such as:

  • Previous psychological, psychiatric, educational, or neuropsychological evaluations

  • Relevant recent treatment or medical records

  • Workplace or school accommodation documentation

  • Disability, leave, or insurance-related forms

  • A list of current medications and treatment providers

You do not need to have every document available before scheduling. We can discuss which records, if any, may be helpful during the consultation. All records need to be uploaded to the secure patient portal after registration (do not email records).

What This Consultation Does Not Include

The Initial Consultation for Psychological Evaluation or Testing is NOT:

  • A psychological evaluation, diagnostic clarification evaluation, or formal testing

  • A written report or completion of workplace, school, leave, disability, or accommodation forms

  • Ongoing treatment

  • A guarantee that further testing, a diagnosis, or documentation will be recommended

  • Legal advice or assistance with disability, employment, benefits, or other claims

Frequently Asked Questions

Is this consultation required before I schedule an evaluation listed on the website?

Not necessarily. If an evaluation type is listed in our scheduler and is available for scheduling online, please feel free to schedule the evaluation directly. However, if you are not sure OR your evaluation type is not available for automatic scheduling, it may be helpful to schedule this initial visit. Below are some of the evaluations you can self-schedule:

Will I receive a diagnosis during the consultation?

Not necessarily. The appointment is intended to clarify your concerns and identify appropriate next steps. A formal diagnosis may require a more comprehensive evaluation, psychological testing, treatment history, records review, or further assessment.

Will psychological testing/evaluation services be scheduled automatically?

No. Further services are recommended only when it is clinically appropriate and likely to provide meaningful information.

Can this consultation help determine whether I need an evaluation service for things like FMLA, short-term disability or accommodations?

Yes. The consultation may help clarify whether a separate and specific type of evaluation is appropriate.

This may include workplace accommodations, academic accommodations, short-term disability, or other documentation-related evaluation needs within our scope of services. If you need a more in-depth evaluation, such as a short-term disability evaluation or workplace accommodations evaluation, we can discuss what that would look like, fees, and timeframes.

Can the consultation help me choose between different evaluation services?

Yes. That is one of the primary purposes of the appointment. We can discuss whether your needs appear to fit diagnostic clarification, workplace or leave evaluation, accommodation testing, treatment consultation, independent evaluation, or referral to another provider.

Can forms or letters be completed during this appointment?

No. The consultation does not include form completion, certification, accommodation letters, disability opinions, or other formal documentation. A separate evaluation may be required.

Do I need to upload records before the consultation?

Not always. You may provide records that are directly relevant to your question. Extensive records review is not included in the standard consultation and may require a separate service or fee.

Is the consultation fee applied to a later evaluation?

We can provide you with a customized fee for the evaluation, depending on what is needed. If we have gathered enough information during the initial consultation, the evaluation appointments may be shorter and we will adjust the fees accordingly. We can provide you with a quote for services in writing (good faith estimate) before scheduling any follow up evaluation visits.

Do you offer online services outside Florida?

Yes. Amica Clinical Consulting can provide online services to adults located in PSYPACT-participating states, including AL, AZ, AR, CO, CNMI, CT, DE, DC, FL, GA, ID, IL, IN, KS, KY, ME, MD, MI, MN, MO, MS, MT, NE, NV, NH, NJ, NC, ND, OH, OK, PA, RI, SC, SD, TN, TX, UT, VT, VA, WA, WV, WI, and WY. Your location and eligibility must be confirmed before services begin.

We cannot address court-ordered clinical needs, custody issues, marriage/family issues, immediate safety concerns, emotional support animal needs, and similar issues.
